But what are they, really?

What Are Exosomes, in Plain English?
Every cell in your body releases tiny messenger particles called exosomes. Think of them as microscopic delivery vehicles – far smaller than a single skin cell – that carry proteins, growth factors, and genetic signals (like microRNA) from one cell to another. Their job is to tell neighbouring cells what to do: repair, regenerate, calm inflammation, or produce more collagen.
In aesthetic medicine, topical exosome serums (sourced from plant stem cells, or laboratory-processed human and non-human cell lines, depending on the product) are applied to the skin immediately after a treatment that creates “micro-access points.” This timing matters enormously, which is exactly where Laser Genesis, PicoGenesis, and microneedling come in.
Why Exosomes Work Best Paired With a Device Treatment
Exosomes applied to intact, undisturbed skin have a much harder job – the skin’s natural barrier is designed to keep things out. But when the skin has just been treated with a laser or fine needling, that barrier becomes temporarily more receptive, and the tissue itself is primed and actively signalling for repair. This is when exosomes have the greatest opportunity to support the skin’s own regenerative response.
This is why, at MediGlow, we rarely recommend exosomes as a stand-alone treatment. Instead, we look at them as a regenerative accelerant layered onto proven energy-based and needling treatments.
Laser Genesis + Exosomes
Laser Genesis uses gentle, non-ablative Nd:YAG energy to heat the deeper layers of the skin, encouraging collagen remodelling, reducing redness, and softening fine lines – with virtually no downtime. Applying an exosome serum straight after a Laser Genesis session gives the skin an additional regenerative signal at precisely the moment it’s most responsive, which many patients find translates into a calmer, faster-settling glow and continued improvement in texture over the following days.

PicoGenesis + Exosomes
PicoGenesis uses ultra-short pico-second pulses to target pigmentation, sun damage, and uneven tone – including in richer, more melanin-dense skin (Fitzpatrick IV–VI), where treatment has to be approached carefully to avoid post-inflammatory hyperpigmentation. Because PicoGenesis creates a controlled inflammatory response as part of how it clears pigment, following it with an exosome application can support a smoother recovery and help the skin settle with less visible irritation – something our South Asian and darker-skinned patients in particular ask us about often.
Microneedling + Exosomes
Microneedling is, quite simply, the most direct delivery method for topical exosomes. The fine needles create thousands of micro-channels that allow the serum to reach deeper into the dermis rather than sitting on the surface. This is why exosome-enhanced microneedling has become one of our most requested combination protocols for patients focused on acne scarring, enlarged pores, fine lines, and overall skin quality.
Is This Right for Everyone?
Not necessarily – and we’ll always tell you honestly if it isn’t. Exosome add-ons work best for patients who are already having Laser Genesis, PicoGenesis, or microneedling for a specific concern and want to enhance recovery and results. They are not a replacement for medical-grade skincare, sun protection, or a proper consultation.
As pharmacist independent prescribers, Pompi and I also look closely at product sourcing and regulatory standing before we bring any exosome serum into clinic – this is a fast-moving, lightly regulated space, and not every product on the market is created equally.
